Key Responsibilities:
- Conduct comprehensive assessments, focusing on physical health, mental health, and substance use.
- Collaborate with a multidisciplinary team, including geriatric psychiatrists and outreach counselors, to deliver holistic mental health care and psychosocial support.
- Engage in community outreach to support medication management, illness self-management, and effective utilization of health services.

Qualifications:
- Current General Class (Practicing) Registration with the College of Nurses of Ontario.
- Experience in psychiatric mental health or addiction nursing, particularly in community settings.
- Familiarity with administering standardized screening and assessment tools.
Preferred Experience:
- Certifications or training in mental health care, gerontology, or working with homeless populations.
- A background in crisis intervention, with strong assessment and collaborative care planning skills.
