An innovative health program, dedicated to supporting older adults facing homelessness or precarious housing situations compounded by chronic mental health challenges, is seeking a Registered Nurse to play a pivotal role in our interdisciplinary team. This program, designed to provide long-term, dignified support beyond traditional short-term solutions, aims to comprehensively uplift those neglected by the health and social care systems.

Key Responsibilities:

  • Conduct comprehensive assessments, focusing on physical health, mental health, and substance use.
  • Collaborate with a multidisciplinary team, including geriatric psychiatrists and outreach counselors, to deliver holistic mental health care and psychosocial support.
  • Engage in community outreach to support medication management, illness self-management, and effective utilization of health services.

Position Overview:

  • Full-time role based in Toronto, offering outreach case management services for homeless older adults with complex needs.
  • Involvement in an innovative program with the support of a dedicated team to find housing solutions, access psychiatric care, and improve psychological and social welfare.

Qualifications:

  • Current General Class (Practicing) Registration with the College of Nurses of Ontario.
  • Experience in psychiatric mental health or addiction nursing, particularly in community settings.
  • Familiarity with administering standardized screening and assessment tools.

Preferred Experience:

  • Certifications or training in mental health care, gerontology, or working with homeless populations.
  • A background in crisis intervention, with strong assessment and collaborative care planning skills.
